Briefly, I 42 and was diagnosed at 22 as having familial hypercholesterolaemia FH ; - inherited high cholesterol from my parents. I struggled to get my cholesterol down with tablets, good diet, exercise, not being overweight and never having smoked. Unfortunately for me when my chest pain was diagnosed as angina at age 34, I was already 99% blocked in all of my coronary arteries. I had a bypass but because things were so advanced I was still left with angina. I had been experiencing chest pain for seven years before my diagnosis. One year on with cholesterol still not under control and my angina becoming more severe, I underwent another angiogram, but this time was inoperable. I continued to decline and finally ran out of options, I thought. but along came LDL-apheresis a procedure similar to dialysis where the LDL cholesterol is removed from my blood every fortnight ; . Four-and-a-half years on and I about to undergo another angiogram with the hope of more interventions but for positive reasons I may have improved enough for something to finally be done and an end to or further improvement in my angina. I will keep you all informed of my progress. This experience, and that of my family's, of cholesterol and how severe heart disease affects every aspect of your life gives me a valuable resource - something I know I could have done with earlier. Please write or email your questions on anything as diverse as heart disease, surgery, LDL-apheresis, medication, diet, children, exercise, benefits system, holidays, insurance, driving or family planning. Really no question is too big or too small. This is for patients and patients decide what they want. My motto is: life is for living." Dawn Davies Ed's note: We were delighted when Dawn volunteered to write a regular column in the Digest from a patient's perspective, in addition to her valued work as a patient representative Trustee of the charity. The Arthritis Foundation did it for a million dollars. Then the American Cancer Society got million for doing it. No wonder the leaders of the American Medical Association AMA ; thought the group could pick up some easy cash by selling its name for "endorsements." What they didn't count on was the tremendous public outcry which met the announcement that, in exchange for an undisclosed portion of the profits, the AMA would "endorse" a variety of home health products made by Sunbeam Corp. The AMA and Sunbeam tried to convince the public that the arrangement was part of a public education program. In a press release distributed by Sunbeam in the summer of 1997, no mention was made of the compensation to be paid to the AMA for use of its name and logo. Sunbeam's home health business includes nine product lines: heating pads, blood pressure monitors, thermometers, air cleaners, vaporizers, humidifiers, scales, hot & cold therapy and massagers. There was no indication that the AMA did any comparison testing to determine the value of these products or their superiority to others on the market. The decision to endorse them was based solely on the amount of money Sunbeam was willing to pay. Al Dunlap, chairman and CEO of Sunbeam hinted at the true motives for the arrangement when he stated, "I expect that this alliance will further enhance our strong competitive position in the marketplace through expanding current distribution and gaining new distribution This co-operative alliance will provide Sunbeam with a dramatic point of differentiation, versus our competition, as we will export the in-store merchandising concept internationally providing further strength to our brand leadership position." Cannabis were further examined and publication is awaited. At the moment, none of the cannabis-based medicines is licensed for medicinal use in the UK although Sativex has been submitted for licence. NICE plans to commence an appraisal process once a licence has been granted. NICE guidance on cannabinoids is significant because it is unlikely that prescription will go ahead on the NHS without its approval. Although illegal, anecdotal evidence suggests many people with MS already use cannabis to control symptoms. A factsheet about cannabis is available from the MS Trust, for example, drug interactions. With those of global radiation, particularly during summer. High correlations between the number concentration of particles larger than 20 nm and the NO concentration were found in winter, indicating that the occurrence of these particles is directly related to emissions from combustion processes. Our parallel measurements have shown that correlation coefficients between two sites only 1.5 km apart are highly size-dependent. Furthermore these correlation coefficients are even modified by changes of typical major sources of particles in this size range. The weakness of the correlation for particle size ranges smaller than 100 nm along with the variability of the correlation coefficients induced by changes of emissions and meteorological conditions poses a major challenge to epidemiological studies investigating short-term health effects associated with size classes of ultrafine particles. Our data suggest that the typical exposure assessment strategy using a single urban background stations may not be appropriate for such studies and carisoprodol. The Alzheimer's Association recently prepared a checklist of the 10 most important warning signs of AD in order to help family members and health-care providers recognize the early onset of the disease. Memory loss. Typical of AD is the tendency to forget recently learned information. Many of us do not recall appointments, names, telephone numbers, where we put our car keys, or exactly where in the huge parking lot we left our cars. If we're not concentrating or are distracted or preoccupied, such memory lapses are understandable. Mucus production in the airways of patients with asthma and chronic obstructive pulmonary disease. Serious side effects from their longterm oral use are much less common when they are inhaled. The four most commonly used agents in the United States for such inhalation therapy are Aerobid flunisolide ; , Vanceril beclometh ; , Azmacort triamcinolone ; , and Pulmicort budesonide ; . There also has been uncertainty about the long-term effect of inhaled corticosteroids on bone health, since these medications may deplete or interfere with the absorption of calcium. That's because when a corticosteroid is inhaled, a small amount is actually swallowed and absorbed through the stomach into the body. Doctors have usually advised anyone who frequently uses this form of therapy to take calcium supplements. The safety of using inhaled corticosteroids during pregnancy has also been questioned because of the suspicion that their use may result in low-birth-weight babies. The motion carried. Reports: Ms. M. J. Terrebonne, Pharmacy Program Director, reported on the status of the Prior Authorization PA ; and Preferred Drug List PDL ; phased-in implementation that began June 10, 2002. Ms. Terrebonne distributed copies of a July 16, 2002 letter to prescribing practitioners and pharmacy providers that notified of the second group of therapeutic classes to be included in the PA process Attachment 1 ; . She then presented a report on PA requests approved for the months of June, July, and August 2002 Attachment 2 ; . She reported a significant increase in PA requests over the months due to the program's phase-in process and to expirations of prescriptions covered by the "Continuity of Care" policy that now require PAs. Note: This policy allows any patient on a current drug regimen, with a prescription issued prior to the effective Prior Authorization date of the drug, to get up to five refills on the prescription within six months of the date the prescription is issued. ; Ms. Terrebonne reported that 5, 930 PA requests were approved in August. Presentation: Mr. Charles Castille, DHH Undersecretary, presented a historical overview on how the State's current purchasing system for Medicaid prescription drugs evolved. He outlined a chronology of the legislation and events that have impacted pricing and delivery of pharmaceuticals from the early 1900s through today Attachment 3 ; . Mr. Castille explained the unique roles various groups, i.e. wholesalers, hospitals, health plans including managed care organizations ; , state governments, the federal government and consumers, play in purchasing pharmaceutical agents and how they impact the way state Medicaid programs currently purchases drugs. He explained that the relationship between a state Medicaid program and drug manufacturers is not direct. He stated that drugs are purchased from pharmacies that provide the prescription services and are reimbursed by states for a prescription's drug ingredient cost plus a dispensing fee. In Louisiana, the maximum dispensing fee is currently .77 although the average dispensing fee is about .54. In 1990 Congress passed legislation known as OBRA 90 which requires manufacturers to have national rebate agreements with the Secretary of the Department of Health and Human Services on behalf of state Medicaid programs. The International Society of Obstetric Medicine ISOM ; was formed two years ago by a group of obstetric medicine leaders from regional societies of obstetric medicine from around the world. ISOM meets every two years in conjunction with the International Society for the Study of Hypertension in Pregnancy ISSHP ; . This year's meeting was held in Vienna November 14-15th. Further information can be found at isshp2004vienna . How does one train in obstetric medicine? Obstetric Medicine has no specialty board or certification. Most of our practitioners have learned their skills through 'on the job training' or obtaining informal training with a colleague already working in the field. However, several formal fellowship training programs do exist. Brown University and Women and Infants' Hospital of Rhode Island have North America's longest running.

Value and limitations of chest pain history in the evaluation of patients with suspected acute coronary syndromes. JAMA, 2006; 295 19 ; : 2248-9 Abstract: - CONTEXT: The chest pain history, physical examination, determination of coronary artery disease CAD ; risk factors, and the initial electrocardiogram compose the information immediately available to clinicians to help determine the probability of acute myocardial infarction AMI ; or acute coronary syndrome ACS ; in patients with chest pain. However, conflicting data exist about the usefulness of the chest pain history and which components are most useful. OBJECTIVE: To identify the elements of the chest pain history that may be most helpful to the clinician in identifying ACS in patients presenting with chest pain. EVIDENCE ACQUISITION: MEDLINE and Ovid were searched from 1970 to September 2005 by using specific key words and Medical Subject Heading terms. Reference lists of these articles and current cardiology textbooks were also consulted. EVIDENCE SYNTHESIS: Certain chest pain characteristics decrease the likelihood of ACS or AMI, namely, pain that is stabbing, pleuritic, positional, or reproducible by palpation likelihood ratios LRs ; 0.2-0.3 ; . Conversely, chest pain that radiates to one shoulder or both shoulders or arms or is precipitated by exertion is associated with LRs 2.3-4.7 ; that increase the likelihood of ACS. The chest pain history itself has not proven to be a powerful enough predictive tool to obviate the need for at least some diagnostic testing. Combinations of elements of the chest pain history with other initially available information, such as a history of CAD, have identified certain groups that may be safe for discharge without further evaluation, but further study is needed before such a recommendation can be considered reasonable. CONCLUSION: Although certain elements of the chest pain history are associated with increased or decreased likelihoods of a diagnosis of ACS or AMI, none of them alone or in combination identify a group of patients that can be safely discharged without further diagnostic testing.
