The assembly speaker V. Tyulpanov reported that in 2006 the federal debt to suppliers equaled 2.2 billion rubles. 400 million had already been appropriated from the state budget to reimburse the distributors; as a result, today's debt totals 1.8 billion rubles. At the same time, it is quite possible that a similar situation that can give rise to a public tumult will be observed in 2008. At present, nearly 70% of the total sum received by Saint-Petersburg from the RF budget is spent for drug aid to a mere 3% of the city's beneficiaries. The maximum amount of money to be spent for drug provision to the city's beneficiaries is fixed at the level of 369 million rubles, while only for the 1st quarter 2007 drugs have been ordered for the amount of 712.3 million rubles. At a meeting of the committee on budgets and finances of the city's government on June 18, Saint-Petersburg's territorial OMI fund executive director V. Kolabutin stressed that an additional sum of 1.37 billion rubles will be allocated to provide drug aid to special population categories. He noted that the territorial fund's budget is planned to be expanded by increasing the size of insurance premiums by 300 million rubles and of social taxes - by 75.92 million rubles. If the amendments are considered, the total territorial OMI fund budget equals 13.76 billion rubles - up 1.89 billion 15.9% of the total budget ; . MCI: some important developments relate to "Mild Cognitive Impairment" MCI ; , a condition that is being increasingly found in relatively non-aged adults. In MCI there is a level of cognitive and memory impairment beyond that expected for normal aging but not bad enough to be called dementia or Alzheimer Disease. However, 10 -15 % of MCI individuals per year go on to develop the full-blown disease. Brain imaging is showing that pathological changes already exist in the brain before the person develops symptoms of Alzheimer Disease. These imaging approaches, added to psychological testing, should therefore make it possible to pick out the most at-risk MCI individuals, in effect making early diagnosis of Alzheimer Disease a reality, at least in the MCI group. The importance of early diagnosis lies in the fact that the best time for treatment is always as early as possible, in Alzheimer Disease as in other diseases. ii ; Statins: these cholesterol-reducing agents are being investigated because the incidence of Alzheimer Disease appeared to decrease in people using this drug for lowering their cholesterol levels. The decreased incidence of Alzheimer Disease could be because lowering the cholesterol reduces the incidence of vascular disease, which is a risk factor for Alzheimer Disease. However, current opinion is that statins could also work by preventing cholesterol from reaching levels that actually interfere in its normal role of helping to maintain the structural and functional integrity of the outer membranes that enclose the nerve cells. iii ; Anti-inflammatory agents such as aspirin and other NSAIDs nonsteroidal anti-inflammatory drugs ; : Although not yet proven, there is intriguing evidence that people routinely taking anti-inflammatory agents decrease their risk of getting Alzheimer Disease, and this lead is being followed up and glucotrol. CLASS: HIV protease inhibitor STANDARD DOSE: Two 250 mg capsules with two 100 mg capsules of Norvir, both twice daily. Take with food. Take missed dose as soon as possible but do not double up on your next dose. AWP: , 072.80 month for Aptivus only MANUFACTURER CONTACT: Boehringer-Ingelheim, aptivus , 1 800 ; 2748651 AIDSINFO: 1 800 ; HIV0440 4480440 ; , aidsinfo.nih.gov POTENTIAL SIDE EFFECTS AND TOXICITY: Mostly gastrointestinal-related: mild diarrhea, nausea, vomiting and fatigue. In clinical trials symptoms have been managed by having a light snack with the drug. Other side effects include headaches, dry mouth, rash, and dizziness. Recent reports of liver problems in people taking it who also have hepatitis. Be sure to know your hepatitis status if you are about to or are taking this drug! During clinical studies, bleeding in the brain occurred in people taking Aptivus Norvir who had medical conditions or were receiving other medications that may have increased the risk of this. Use with caution by people who may be at risk of increased bleeding from trauma, surgery or other medical conditions, or who are receiving medications known to increase the risk of bleeding such as antiplatelet drugs or anticoagulants. Aptivus has a "sulfa" component to it, so it should be used cautiously in patients with "sulfa" allergies. As seen with other protease inhibitors, there can be increased levels of cholesterol and triglycerides except possibly unboosted Reyataz ; which may be associated with an increased risk of heart disease. Other possible side effects are lipodystrophy body fat changes, including thinning of the face, arms and legs, with or without fat accumulation in the stomach, breasts and sometimes the upper back ; , onset of new cases or worsening of diabetes see your doctor promptly ; , and increased bleeding in hemophiliacs. See Norvir for more details on potential side effects. POTENTIAL DRUG INTERACTIONS: Aptivus Norvir interacts with many other drugs, so it is important to tell your healthcare professional of the medications you are taking. See the manufacturer package insert. Do not take with Tambocor, Rythmol, Cordarone, quinidine, Versed, Halcion, Rifadin, Orap, ergot derivatives such as Cafergot, Wigraine and Methergine, D.H.E. 45 ; , or the herb St. John's wort. Do not use Zocor or Mevacor; lipid-lowering alternatives are Lipitor, Lescol, and Pravachol, but they should be used with caution due to potential for liver toxicity. Increased levels of the inhaled and nasal sprays with fluticasone found in Advair, Flonase, Flovent ; , can occur with Aptivus Norvir and therefore should be used with caution. This drug is metabolized by the liver same as most of the other protease inhibitors ; . Should not be given with other protease inhibitors because it greatly lowers their blood levels. Cialis, Levitra, and Viagra levels are increased; doses should not exceed 10 mg Cialis per 72 hours, 2.5 mg Levitra per 72 hours, or 25 mg Viagra per 48 hours. Norvir may decrease levels of methadone, but withdrawal rarely occurs. Methadone doses may need to be increased. A lower dose of Desyrel is recommended. The blood pressure medications called calcium channel blockers such as Norvasc, Procardia, and others should be monitored for side effects. Monitoring may be required when taking Coumadin or immunosuppressants. Tegretol, Dilantin or phenobarbital may decrease Aptivus, so alternate seizure medications should be used and monitoring of Aptivus drug levels is recommended. Caution must be exercised when using Sporanax or Diflucan fluconazole ; . Rifabutin requires a reduced dose. The Norvir and Aptivus capsules contain alcohol but should not be enough to trigger relapse ; , so be cautious with Antabuse or Flagyl, which can cause flushing, vomiting, etc. TIPS: Take with food to minimize stomach problems. Do not take at the same time as antacids. This drug does its best when used with T-20 enfuvirtide, Fuzeon ; . Unlike adding 1 + 1 2, with Aptivus and Fuzeon, 1 + 1 3! drug is only for experienced patients or those with resistance to multiple protease inhibitors. Tipranavir is expected to do less well for people with combinations of certain protease-related mutations. See package insert or aptivus for a list of mutations. Although tipranavir has to be taken with 200 mg twice daily of Norvir, it actually lowers the blood levels of Norvir. So, you may not see as much of the GI side effects as you might expect. The capsules should be refrigerated prior to opening. Once the bottle is opened, Aptivus can be stored at temperatures less than 77F and must be used within 60 days.--Patrick G. Clay, Pharm.D. Data on that. This is not a significant issue for me, but others disagree." Genentech earned praise for its reimbursement program. A source said, "Genentech has done a great job with reimbursement. They have a HIPAA form patients sign that lets the Genentech sales rep see the chart. And they have a great needle collection program." Following are some of the comments by speakers and other doctors about Raptiva: Thrombocytopenia. "Thrombocytopenia is a very, very small risk.Thrombocytopenia may or may not be a real phenomenon, but you are being asked to monitor for that monthly, at least in the beginning." Rebound. Several speakers emphasized that Raptiva "should never be discontinued abruptly." One said, "You need an exit strategy. The rebound issue may be minimized by using combination and sequential therapy with a lot of tapering rather than abrupt discontinuation." Another commented, "When patients are coming off Raptiva, you need to be careful.Flare is more likely to occur in patients with poor response to Raptiva. I suggest Raptiva be tapered by overlapping with topicals, UVB or Soriatane." A third said, "Rebound is a concern. A significant number of patients 13.8% flare upon discontinuation." A fourth speaker said, "Median time to relapse is 64-70 days, which I would call a soft landing.but 13.8% get a little worse after stopping therapy, which is most likely to occur in patients who did not achieve PASI-50.This drug should not be discontinued without a second drug being available." Another expert said, "Patients who discontinue have had severe, profound flares. That is not acceptable in an environment where patients for managed care or other reasons might need to interrupt therapy abruptly.Patients on systemic corticosteroids who stop can convert to pustular psoriasis, which can be potentially life-threatening, and we need to be sure that this doesn't happen with Raptiva ; ." Malignancy. "Malignancies do not appear to be increased, but non-melanoma skin cancers were seen 20 observed cases vs. seven expected ; ." Antibodies. "Antibodies form 6.3% incidence ; , but that doesn't affect the PASI score." Pregnancy. This agent has the worst pregnancy label Class C ; , but speakers emphasized that only Genentech did animal pregnancy studies, so no one really knows the teratogenicity of the other biologics. One speaker said, "For all intents and purposes Raptiva can be considered a Category B for pregnancy.I met yesterday with someone who ran a pregnancy registry in oncology for liver transplants.and we included some of our cyclosporine pregnancies to show no increased risk of teratogenicity with these drugs. So, I hope in the years ahead, as the companies keep building data, they will open a huge new market for women of childbearing age." Another speaker said, "Raptiva has an issue with the mouse immune system, so it got a worse category.I wouldn't want.

If you either cannot or do not want to meet in person, present your concerns in a letter. This approach can be useful because it gives your provider time to think about your concerns before getting back to you. Ask for your health care provider's ideas about ways to resolve the problem. If you are generally satisfied with your health care provider, but have concerns about a particular decision or recommendation he or she has made, you may want to get a second opinion about that particular issue. Your health care provider can give you the names of other providers who can provide a second opinion. Under some managed care plans, you may have to pay for a second opinion. However, you may be able to appeal this decision. It is important to try to work through any difficulties you might have with your health care provider. However, if it appears that it is just not a good match, you may want to consider finding a different health care provider. This is your right as a patient. Your health care provider should be willing to give you a referral to another health care provider if you request it. You can also get referrals from other health care providers or other people with hepatitis C. You may need to get approval from your insurance carrier before changing care providers. Though it can be somewhat stressful, keep in mind that changing health care providers does not mean there is anything wrong with either you or your health care provider. Your Role in Treatment: Gathering Information You can take an active role in your health care by educating yourself about hepatitis C. Your health care providers may have patient education materials available about your medications or supplements, their side effects, and how to manage them. Since product manufacturers often provide these materials, you might ask your health care provider to direct you to additional sources of information. Patient advocacy organizations and hepatitis C support groups can be invaluable sources of information about the disease and its treatment. See the Resource Directory at the back of this manual for a list of hepatitis C advocacy and support organizations. If you are comfortable reading technical information, you may want to read medical journal articles about hepatitis C. However, be aware that these journals are intended for health care professionals, so they can be very challenging reading. If you are interested, ask a member of your health care team for recommendations about appropriate journals and or articles. A hospital librarian or the reference librarian at your local public library can help you locate specific journals or articles. The Internet is another good resource for information about hepatitis C. Any search engine can lead you to a number of resources. However, keep in mind the Internet is unregulated. Anyone can post just about anything on the Internet regardless of its accuracy. Sites that are reviewed by a medical advisory board are generally more reliable sources of information than sites with no such oversight. Getting Information from Your Health Care Providers Gathering information takes time and effort. However, it is important to get answers to your questions. If you have questions for your health care provider, organize and write them down ahead of time. Your time with your health care provider is often limited. Being organized will help you get as many of your questions answered as possible in the time available. Identify the two or three most urgent questions and ibuprofen.
