Discussion Reports of dizziness or lightheadedness requiring medical attention have been published for salbutamol, bitolterol and fenoterol [5]. However vertigo and dizziness have been considered to occur only very rarely in association with salbutamol [6]. In any case, dizziness is thought to be a cardiovascular-related effect of selective 2-agonists due to 1-receptor stimulation, and can be explained in the context of hypotension that is accompanied by dizziness and fainting [7]. However, episodes of persistent vertigo such as those experienced by this patient ; might be explained by another mechanism of action. The Commission has taken account of the fact that demand for prescription drugs is generated by prescribing doctors and not patients. The Commission considers that "interchangeability" of prescription drugs does not depend on their physical, technical or chemical properties, but on their "functional substitutability as viewed by those supervising their consumption.84 However, prescriptions are established according to the practitioner's knowledge of the active properties and similarities of different medicines, and not necessarily in terms of therapeutic indication only. Carrying out an audit or introducing a `modernised' style of service are automatically noted as positive, but continuing to see difficult patients who complain, don't get better and generate `untoward incidents' may even downgrade you in your chief executive's eyes.
